Terms & Consent for Massage & Spa Treatments: You must be over 18 years of age and submit accurate information about yourself in order to schedule a massage or spa treatment. The massage you receive will be provided by a Trained Massage Therapist and is only for the basic purpose of relaxation.
If you experience any pain or discomfort during the massage session, or spa treatment, you understand it is your responsibility to immediately inform the person giving the treatment so that the treatment can stop or pressure and strokes may be adjusted to your level of comfort.
You further understand that the person giving the massage, or spa treatment, is not an esthetican and has no professional training or license for hair or skin care. Also person giving treatment, has no professional medical training and should not be construed as a substitute for medical examination, diagnosis, or treatment. You also understand that a person giving massage is not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat any physical or mental illness, and that nothing said in the course of the session given should be construed as such. Because massage should not be performed under certain medical conditions, you affirm that you have stated all your known physical and medical conditions to the person giving your massage before the massage begins and will disclose all information and answer all questions honestly.
You agree to keep the person giving the massage or spa treatment updated as to any changes in your medical health profile (now and between future sessions) and understand that there shall be no liability on the part of the person giving your massage or spa treatment. You agree that the person giving the massage or spa treatment will not in anyway be held responsible for injury before, during or after your massage or treatments.
You also understand that the person giving your massage & spa treatments reserves the right to refuse to perform services if he thinks you may have a condition for which the treatment is contraindicated. Any illicit or sexually suggestive remarks or advances made may result in termination of the treatment session. If you do not agree with these terms, please do not schedule a massage or spa treatment appointment.
It is your responsibility, as the person receiving the treatment, to inform the person giving the treatment if there have been any health changes or injuries since your last treatment. Also it is your responsibility to check these terms of massage, before each future treatments, as these terms may change from time to time. The email address you submit may be used to communicate with you in the future regarding updates and new information.
Important Payment Information: Cash or credit card payment at time of massage is accepted, sorry no checks. If paying on-line with a credit card or PayPal—only make payment after completing the intake form and confirming your massage appointment date and time by e-mail. Refunds may take up to two weeks. NOTE: There is a $25 fee if you don't show for your appointment or cancel with less than 24 hours notice.
